Nominations will be accepted from anyone living with MND or their carer or a family member.
Nominees should be working in England, Wales or Northern Ireland.

Fill in a nomination form

Fill in an EMAS Nominaton Form (1657 kb) and post it back to EMAS, MND Association, PO Box, 246, Northampton, NN1 2PR.
