Thank you for submitting your photos for use by the MND Association. We’re creating a unique film featuring people affected by motor neurone disease (MND). Working in partnership with our agency partners Strange Weather Ltd, our goal is to raise awareness of MND and the work of the MND Association through authentic stories and moments from the MND community.

By agreeing to the MND Association's conditions of use, I am permitting The Association to use my photograph/s within their publications and presentations, across their digital and social channels, and within their internal and external videos at any time with a five-year period of today’s date. I am agreeing to my name being used alongside the photograph/s I have submitted. I can confirm I have been granted permission for the image to be used by the person/s within the image. I understand that any photographs or film will be processed in accordance with the Data Protection Act 2018, the General Data Protection Regulation and any future legislation. I understand that the material produced may be used more than once, without restriction and without compensation to me. The MND Association will not publish your contact details in our promotional publications and presentations, or within our digital channels and promotional videos. You can withdraw consent of the use of your image at any time. For details on how we use your information, please see our privacy policy which can be found on the home page of our website.
I agree that the MND Association’s partner on this project, Strange Weather Ltd (“the Company”), along with its legal representatives, partners, and anyone working under its permission or authority, may film, record, photograph, and use my image, voice, and performance. The MND Association is the data controller for this project. Strange Weather Ltd acts as a contracted processor to produce the film under the Association’s instruction. The Company may use this material in full or in part, and it may be edited, altered, or combined with other material. It may be used by the MND Association and Strange Weather Ltd for the purposes of this project and related awareness activities, in any media format now known or created in the future, anywhere in the world, for up to five years from the date of this agreement. After that period, the material will no longer be used in new content unless renewed consent is obtained.
I understand that whilst the The Company will endeavour to show me the film ahead of release, I won’t have the right to review or approve the finished product before it is used.
I confirm that anything I say on camera does not break any laws or violate anyone’s rights, including privacy or copyright. I release the Company and its partners from any legal responsibility for how the material is used, including if my image or voice is altered, edited, or combined in a way that changes how I look or sound. I also waive any claims I might have in the future about invasion of privacy, defamation, or misuse of my likeness, voice, or performance.
I understand that I will not receive any payment (and neither will my company, if applicable) for the use of this footage, audio, or transcript. I give up any right to payment now or in the future for this use.
I confirm that I am over 18 years old and have the legal right to agree to this release. If I am under 18, my parent or legal guardian must sign on my behalf.
I have read and understood this release before signing it, and I agree that it represents the entire agreement between me and the Company.
